Guide to Registration and Advance Appointments        

 

I. Same-day Appointments

1. Same-day appointments should be made 8:10~11:30 AM for morning clinics, 1:00~ 4:00 PM for afternoon clinics.

2.Same-day registration for return visits can be made by presenting your NHI   card and passport.

 

II. Registration for First-visits with Advance Appointments

1.A “First Visit Registration” form should be filled out before registration.

2.Indicate the department and the name of the doctor at registration. A registration receipt will be given.

3.Documents needed: passport or Alien Resident Certificate, NHI card.

4.Upon arrival at the clinic, the registration receipt should be placed in the slot for the registration receipt next to the examination door.

5.Dental patients should report to the dental registration and cashier counter in the dental clinic on the second floor.

6.Patients seeking nutrition consultation should register at the registration counters on the first floor before reporting to the clinic.

 

III. Registration for Return Visits with Advance Appointments

1.Report directly to the clinic on the date of the appointment.

2.Patients who are NHI insured should place Registration Card of NCKUH, NHI card, passport or Alien Resident Certificate in the slot next to the door of the examination room.

 

IV. Making Advance Appointments

1.Advance appointments for outpatient clinics can be made five weeks

beforehand in the following 5 ways:

(1)Advance appointments can be placed at the registration counters.

(2)Appointments made in the clinic: doctors will make appointments for returning visits if necessary.

(3)24-hour appointments via voice system: (06) 2766111.

(4)Appointments via phone: (06) 2353333

7:30 AM ~5:30 PM, Monday through Friday

8:00 AM ~12:00 AM, Saturday

2. Advance appointments can be canceled by 7:00 AM for morning clinics, 12:00  PM for afternoon clinics.

3. If you don’t show up for advance appointments twice in three months, you lose the right to make further advance appointments.

 

V.Cancelling appointments

1.          Same-day appointments can be cancelled before visit at the registration counters during office hours on the same day of appointment.

2.          Advance appointments can be cancelled in the following ways up to the day before the appointment:

(1)  Cancel appointments via the phone: (06) 2353333.

7:30 AM ~ 5:30 PM, Monday through Friday.

8:00 ~12:00 AM, Saturday.

(2)  Cancel appointments via the voice system 24 hours: (06) 2766111.

(3)  Cancel appointments via internet:

Ⅵ.Referral Registration should be made at the Referral Center located in the lobby on then first floor.

 

Ⅶ.Registration fee:

Self-pay patients: NT$ 300~330 (NT$120 for registration and NT$ 180 for doctor fee.)

NHI-insured patients: NT$120 plus NT$ 210 (NHI copayment).

 

 

 

Guide to Physical Examinations

 

1. Place for registration: the Referral Center on the first floor.

2. Registration hours: 8:30~10:30 AM, Monday through Friday.

3. Daily Maximum: 25.

4. Location for physical examinations: Clinic 6, Family Medicine, second floor.

5. Documents needed: ID card, passport, NHI card, and two photos.

6. Fees vary as items required differ.

7. To pick up examination results:

(1) Chinese copies of examination results of basic items can be picked up on the same day of examination. More comprehensive copies that include other results such as X-ray and blood test results can be picked up one week after the examination.

(2) English copies can be picked up two weeks after the examination.

 

 

 

Guide to Applying for Diagnosis Certificates

 

 

1.  Qualifications: Only the patient, his spouse, or his immediate family can apply.

2.  Documents required: ID card (passport or Alien Resident Certificate) of the patient, and ID card (passport or Alien Resident Certificate) of the charge d’affaires.

3.  Register to see the original doctor for diagnosis certificates, which should be stamped by the doctor.

4.  After obtaining the diagnosis certificate, pay at the cashier counters, and have the diagnosis certificate sealed at the Official Seal Office on the first floor.
